Assertive Community Treatment Team (ACTT)

The Assertive Community Treatment Team serves individuals in their late teens to their elderly years that have a severe and persistent mental illness. ACTT uses a team approach to be available 24/7 with rapid response to calls, while working with individuals in their home, at school or at work. ACTT strives to lessen or eliminate the debilitating symptoms of the mental illness each client experiences and to minimize or prevent recurrent episodes of the illness, to meet basic needs and enhance quality of life, to improve functioning in adult, social and employment roles, to make them capable of living independently in their own community and to lessen the family’s burden of providing care. The main goal of this intensive service is to help individuals remain out of the hospital and living in the community.

Community Support Team (CST)

This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual’s ability to live successfully in the community.

Diagnostic Assessment / Clinical Comprehensive Assessment

Diagnostic Assessments are in-depth evaluations of a person’s current life situation, including functioning level, symptoms and/or problems, and personal history. These assessments screen for mental health conditions, purpose possible follow-up procedures, and provide a written summary of findings, with recommendations as necessary. Diagnostic assessments are the first step in determining what services may assist individuals and/or children in meeting their specific needs.

Laboratory

Our CLIA certified laboratory provides comprehensive testing done by compassionate staff, when and where you need it to monitor medication management and compliance.

Medication Management / Evaluation

Medication Management serves individuals who are currently taking prescription drugs for the treatment of a mental health diagnosis. This service allows individuals to meet regularly with a doctor to discuss the effectiveness of current medications. Medication Evaluations are available to consumers who are not currently on prescription medications. With a doctor, individuals assess possible benefits of medication during treatment of a mental health diagnosis.

Outpatient Therapy

We offer both individual and family therapy. We have several counselors to choose from, male and female, with expertise in child mental health and adult mental health treatment modalities. Our therapists are Licensed Clinical Social Workers and Licensed Professional Counselors.

Peer Support

Peer Support Services are recovery-oriented services provided by North Carolina Certified Peer Support Specialists who use their lived experience to support individuals in achieving personal recovery goals. Peer Support Specialists offer encouragement, skill-building, and connection to community resources while promoting hope, self-advocacy, and wellness. These services complement clinical care by helping individuals build confidence, resilience, and long-term recovery supports.

Tailored Care Management (TCM)

Tailored Care Management (TCM) is a person-centered service that helps individuals with complex behavioral health, physical health, and social needs coordinate their care. Through TCM, care managers work closely with individuals to assess needs, develop care plans, and connect them to medical, behavioral health, and community resources. This service supports whole-person care and helps individuals navigate systems to improve health outcomes and quality of life.

Therapy - Individual + Family

Pathways offers one-on-one, family, or group sessions with professional staff to develop coping skills and nurture overall wellness in all areas of mental health.
